Risk Management Forms
The Office of Human Resources and Risk Management provides the following forms to assist employees and supervisors in reporting work-related injuries and incidents in a timely manner. Prompt reporting helps ensure appropriate medical care, accurate documentation, and compliance with Workers' Compensation requirements.
Available Forms:
Supervisor's 1st Report of Work-Related Injury - Completed by the supervisor to document the initial details of a work-related injury or illness and initiate the Workers' Compensation reporting process. This report should be submitted to Human Resources within 24 hours of the employee reporting the injury.
Supervisor's Incident & Injury Report - Completed by the supervisor to document the incident, identify contributing factors, determine root causes, and recommend corrective actions to help prevent similar incidents in the future.
Employee's Incident & Injury Report - Completed by the employee to provide a first-hand account of the work-related incident or injury, including what occurred, any injuries sustained, and relevant witness information. Employees should complete and submit this form to their supervisor as soon as possible following the incident.
Submission Instructions
These forms must be completed promptly to ensure proper documentation, timely response, and compliance with City reporting requirements. Please submit all completed forms to the Office of Human Resources and Risk Management as required.
